Risk Questionnaire

I am 45 years of age or older and I am 25 pounds or
more overweight.
2.
I have a sister or brother with diabetes.
3.
I have a mother, father, aunt or uncle with diabetes.
4.
I am a woman who has had a baby weighing more than nine
pounds at birth.
5.
I don't exercise at least five times a week for 30 minutes
duration.
If
you answered yes to two or more questions, we invite
you to schedule an appointment for testing your hemoglobin
A1C at the upcoming health fair. Our experienced nurse
will prick your finger for the tiny drop of blood that
is needed for this very useful blood test. The test
will help determine if your blood sugar level has been
elevated in the past 2-3 months. The results are available
in less than 10 minutes and the nurse will provide you
with information on what to do with your results. You
can eat normally the day of this blood test; no fasting
is required.
Heart
Disease Risk Questionnaire

I smoke cigarettes.
2.
I have a family history of coronary heart disease (father
or brother with heart attack or angina before age 55
or a mother or sister with heart attack or angina before
age 65).
3.
I have a history of high blood pressure, or I take medication
regularly to control my high blood pressure.
4.
I don't exercise at least five times a week for 30 minutes
duration.
5.
I am a woman who has gone through menopause or has had
ovaries removed.
6.
I am 20 pounds or more overweight.
7.
I have elevated cholesterol and or LDL levels, or I
take medication to control my cholesterol levels.
If
you answered yes to three or more questions, we invite
you to schedule an appointment at your health fair for
testing your total cholesterol and HDL blood levels.
Our experienced nurse will prick your finger for the
tiny drop of blood that is needed for this very useful
blood test. The results are available in less than 10
minutes and the nurse will provide you with information
on what to do with your results. A 12-hour fast is required
for this test. We will make every effort to schedule
your test early on the morning of your health fair.
